Timesheets

Please be advised as follows:
  1. All timesheets must be signed by the caregiver working the shift.
  2. All timesheets must be signed by the patient or the patient’s representative, unless the office has instructed differently.
  3. For each task you are expected to perform based upon the Aide Plan of Care, you must indicate either that you performed it (with a check mark) or that the patient refused (with an “R”).
  4. All timesheets must be neat and legible. They are documents that will become part of the patient’s permanent record and they are subject to audit by regulators. Only blue or black ink must be used (i.e., no pencil). Do not scribble on them or use whiteout. If you must make a minor correction, cross through with one line and initial next to your correction.
  5. Copied timesheets are not acceptable. A new timesheet must be filled out each and every week that you work.
  6. It is YOUR responsibility to turn in your timesheets by the deadline set by the office. If your timesheets are not turned in on-time, payment to you for the time you worked may be delayed until the next payroll cycle.
