These are unprecedented times that call for social distancing and other infection control precautions. To help protect applicants and associate staff, we have developed this platform that allows for nearly all of our on-boarding requirements to be completed remotely. You will still have to come to the office to complete a few remaining items in-person (e.g., let us witness your signature, complete skills testing, obtain your ID badge, etc.), but the your face-to-face interaction will be expedited... we should be able to get you in and out the door quickly and on to your first case! Below is a high-level description of the on-boarding steps to help you navigate where you are in the process and what to expect. Step 1
Click the "Let's Get Started!" button below and complete the forms. Note that there will be some forms that you will sign electronically and others that you will sign later when we see you. By the time you submit, you will have already completed the Employment Application, Affirmative Action Questionnaire, Compliance Attestation, Emergency Contact Form, CHRC Submission Form, and DOH CHRC-102 Consent Form. You won't see it, but on our end, we will receive not just these documents but other documents needed (e.g., reference forms, ID card templates, Live-In Agreement, Conditional Offer of Employment, and Wage Rate Acknowledgement) that have been pre-populated for either follow-up by the Human Resources Department or your express completion later. The completed packet that we receive will look something like this.

Step 2
Once you complete Step 1 by submitting your forms, you will receive an email acknowledgment. Attached to that email will be a handful forms that you need to print and complete, or get completed, before we see you. They include the IRS Form I-9, W-4, Health Assessment, and TB Consent. If you can't get all of these completed, please just do what you can. The email will also include: 1) links for you to download the apps you will eventually need as a caregiver (e.g., HHA eXchange, Relias Learning); and 2) a link to a quiz you will need to take as part of our hiring process if you are already certified as an aide. Step 3
You're almost there! We will send you log-in credentials to complete your initial orientation online and we will schedule a date and time for you to come to the office so we can meet you, finish up your on-boarding, and review expectations. Sounds like a lot, but we can't wait to have you come aboard! |
